The 42 North Dental Cavity Prevention Plan(TM) is a limited-time offer available only at 42 North Dental-supported practices and is subject to all applicable federal, state, and local laws and regulations. Eligibility is limited to tooth surfaces diagnosed by a 42 North Dental-supported dentist as having an incipient lesion or white spot lesion appropriate for treatment with Curodont™. Such dentists shall exercise their independent professional judgment when diagnosing lesions and developing and implementing treatment plans. Curodont™ treatments furnished by dentists outside of the 42 North Dental network are not eligible for coverage under the Prevention Plan.
The lesion must be identified, treated with Curodont™, and the patient’s financial responsibility paid on the same date of service. Activation of the Prevention Plan requires compliance with all dentist-recommended preventive care, including maintaining appointments for examinations, hygiene visits, diagnostic imaging, and home care.
The Prevention Plan applies only to the specific area of the tooth treated with Curodont™ and is void if appointments are missed, care is not followed, or if restorative needs arise from trauma, fracture, or non-decay causes. If all conditions are met and the treated tooth surface develops a cavitated lesion within 24 months of application, a single surface conventional filling (restoration) will be provided at no cost to the patient. This offer has no cash value, is non-transferable, and may be modified or discontinued as permitted by law.
